payment of legal fees in cases where fees are determined based on a flat fee or
an hourly fee. Credit cards will not be accepted, or, if accepted, a surcharge
of 3% will be imposed, for payment for costs expended or toward a contingent
fee, fee awarded by a court, or fees paid pursuant to a negotiated case
